Acting on behalf of MaForm Holding AG (6340 Baar, Neuhofstrasse 25, Switzerland), a company entered in the commercial register of the canton of Zug under no. CHE- 109,290,158

• a shareholder of FABRYKI MEBLI "FORTE" S.A. (the Company), I request that the following items be placed on the agenda of the next Annual General Meeting of the Company concerning the adoption of resolutions on the following matters:
• 1. increase in the number of Members of the Supervisory Board of the current term of
     office,
  2. appointment of a Member of the Supervisory Board of the current term of office,
  3. election of the Chairman of the Supervisory Board of the current term of office,
  4. changes to the remuneration of Members of the Supervisory Board.

Reasons re 1-3 above

Mr Maciej Formanowicz, President of the Management Board of the Company, has resigned from his position on the Management Board as of 31.08.2023 and, at the same time, informed that he intends to stand for election to the Supervisory Board of the current term of office. In view of the above, MaForm Holding AG, in which 100% of shares are held by Mr Maciej Formanowicz, applies to expand the composition of the Supervisory Board to seven persons and proposes Mr Maciej Formanowicz as a candidate for Member of the Supervisory Board starting from 01.10.2023 (ultimately also to perform the function of the Chairman of the Supervisory Board in connection with the resignation of Mr Zbigniew Sebastian). Mr Maciej Formanowicz has many years of experience and extensive knowledge of the Company and the industry in which the Company operates. Thus, increasing the number of Members of the Supervisory Board and electing Mr Maciej Formanowicz will strengthen the work of the supervisory body, as well as ensure strengthening of shareholder supervision over the Company.

Considering that the remuneration of Members of the Supervisory Board was last updated in 2015 on the basis of resolution of the Annual General Meeting no. 23/2015 of 19.05.2015, taking into account resolution of the Annual General Meeting no. 19/2021 of 16.06.2021, under

which only the remuneration of the Chairman of the Audit Committee was increased, I propose that the monthly remuneration starting from 01.10.2023 be as follows:

• Chairman of the Supervisory Board - PLN 12,000 (change from PLN 7,000),
• Vice-Chairmanof the Supervisory Board - PLN 10,000 (new item),
• other Members of the Supervisory Board - PLN 7,000 each (change from PLN 5,000).

I also propose that the remuneration for Member of the Supervisory Board acting as Chairman of the Audit Committee be set at PLN 10,000 (change from PLN 7,000). In the event that the Chairman of the Supervisory Board or the Vice-Chairman of the Supervisory Board becomes the Chairman of the Audit Committee, this will not result in a change in their remuneration (they will be entitled to PLN 12,000 and PLN 10,000, respectively). In the event that a Member of the Supervisory Board becomes the Chairman of the Audit Committee, this will mean an increase in his/her remuneration to PLN 10,000. The proposed changes are intended to take into account the current situation, in particular a significant increase in inflation, as well as increased responsibilities and duties of Members of the Supervisory Board in connection with amendments to the Code of Commercial Companies.

This application is accompanied by:

1. share certificate confirming that MaForm Holding AG is a shareholder of the Company,
2. extract from the register of MaForm Holding AG.
